Are you a Registered Nurse interested in an exciting role that involves mentoring a growing team to deliver case management under the home care package program for people over 65 years? This key leadership position is essential in leading the clinical governance framework and case management of home and community care services.

About the Role:

We are seeking a qualified Registered Nurse to lead, grow, and manage a team of Support at Home Partners (case managers), who provide case management across the home care package program. This position is recognized as a key senior leadership role within the team and will lead the Clinical Governance Framework under the home and community care program.

The Clinical Manager will be responsible for overseeing the day-to-day operations of our home care package program. The position will manage and grow the program, ensure compliance with the Aged Care Quality Standards, and be a key lead in the Clinical Governance Framework. The successful applicant will work closely with Team Leaders and Coordinators to champion high-quality and safe practice, supporting continuous improvement. The successful applicant will also lead, coach, and mentor their team to deliver quality outcomes for clients and build a strong and positive culture in line with our Purpose and Mission – Connecting people to enrich lives and provide services that enhance quality of life.

The Support at Home – Clinical Manager will have the support of the Executive and Leadership teams to guide and support the program's strategic direction and yearly KPI’s.

  • Permanent full-time position (76 hours/fortnight, core hours are 9.00am to 5.00pm)
  • Annual salary commensurate with qualifications and experience, (Social, Community, Home Care and Disability Service Award) plus superannuation and tax benefits to maximize take-home pay

Experience Requirements:

Qualifications and Experience:

  • Registered Nurse with current AHPRA registration
  • Demonstrated experience or knowledge of clinical support and/or case management experience in home care packages or aged care to support complex clients
  • Comprehensive knowledge and understanding of the Aged Care Quality Standards, Clinical Governance requirements, and the aged care reform agenda and funding
  • Demonstrated experience managing daily operations of aged care service programs or similar, including financial management, to deliver strategic outcomes and measurable operational improvement
  • Demonstrated experience in developing and managing teams to deliver high-quality service, and creating a culture of continuous improvement and empowering teams and individuals to speak up when they see opportunities for improvement
  • Excellent written and verbal communication skills, including experience writing reports, policy and procedures, delivering promotional material, and mediating positive outcomes for clients and teams
